This document contains important information about Crutchfield Psychology’s professional services and business policies. It is important that you read this carefully so that Dr. Crutchfield may answer any questions you may have prior to working together.
Professional Services Not Offered
NOTE: Dr. Crutchfield is not a medical doctor. She does not prescribe medication.
Please know that Dr. Crutchfield’s practice focuses on behavioral health counseling. As such, Dr. Crutchfield does NOT perform the following services:
- Formal psychological evaluations.
- Disability determinations or evaluations.
- Independent evaluations for emotional support animals.
Appointments and Fees
Counseling sessions are typically 45 minutes long, though some insurance policies allow for longer sessions. Sessions are scheduled by appointment at differing times during the week. Payment is expected in full at the time of service unless otherwise previously arranged.
Insurance Accepted
Dr. Crutchfield accepts self-pay and is also paneled on most local insurance plans including:
- United
- Medicare
- Aetna
- Humana
- Tricare
- Beacon Health Options
- PHCS
- Out-of-network Blue Cross Blue Shield
Other Professional Fees
- There is a $35.00 fee for all returned checks.
- Please know that there is a charge for time spent on other professional services you may request of Crutchfield Psychology, including report writing, completion of special forms, telephone conversations lasting longer than 10 minutes, consulting with other professionals with your permission, and preparation of records or treatment summaries.
- If you become involved in legal proceedings that require Dr. Crutchfield’s participation, you will be expected to pay for all of Dr. Crutchfield’s professional time, including preparation and transportation time, even if Dr. Crutchfield is called to testify by another party. Because of the difficulty of legal involvement, Dr. Crutchfield’s rate is $250.00 per hour for time spent in preparation, travel and attendance at any legal proceeding.
Late Cancellation and No-Show Policy
- If you wish to cancel or reschedule an appointment, it will be necessary for you to provide 24 hours notice.
- If you fail to show up for a scheduled appointment, you will be responsible for payment of the full appointment fee (not just your copay), as insurance companies and employee assistance programs typically do not cover missed appointments.
- Dr. Crutchfield does not make confirmation calls to clients, so it is fully your responsibility to remember your scheduled appointment.
- Two consecutively missed appointments without contact from you constitutes grounds to terminate our professional relationship.
